Oops!
I forgot to mention that you really should re-check the phase output
lines under power (again, as described in Dan's instructions)with a
meter to be sure they are switching as they should.
This will tell you if a transistor is REALLY blown (usually the magic
white smoke is out by then, though). :)

P.S. It just occurred to me that the translator chip sending the
signals to the output transistors could also be a problem. Not as
likely, but possible. It is also static/heat sensitive.
