Bi-polar driver board help

I need your guy's help. I am trying to build a driver board for some
bi-polar motors that I have. I have bulit a board but it is not
working. I have basic electronic knowledge but not enough to trouble
shoot this board. can someone please help me with this?

using for a bi-polar driver. The other two files are PDF's of the
