technique fails.
Things change. If someone writes a controller program with these background
"calls" or possibilities in mind, the likely hood of accidents will be minimal.
I think Accidents still come back to the operator far more than any out of
control machine. Add to that, if you put a machine in the middle of a "Day
Care", you can clearly expect your accident odds to change. Point being, the
AREA around the machine must allow for safe operation.
