Can someone make me a map of which pin goes to which terminal?

The board is from deskamnc.

This is the exsisting pin out set up to drive the controller.

Pin 15 is used to indicate to the Servo Fault condition.
Pin 2 input X axis step.
Pin 3 input X movement direction.
Pin 4 input Y axis step.
Pin 5 input Y movement direction.
Pin 6 input Relay A on when high.
Pin 7 input Relay B on when high.
Pin 8 input Z axis step.
Pin 9 input Z movement direction.
Pin 11 output X limit switch.
Pin 12 output X limit switch.
Pin 13 output X limit switch.
Pin 14 input START
Pin 15 output Servo Fault
Pin 16 input must be high
Pin 17 input STOP
Pins 18 through 25 are ground, signal return.

Here is the link with the diagram for the port.

I understand the step and direction wires .
I dont understand where the 5v comes from and the ground .
The board has 5v beside each step and direction set up and a ground
terminal. Im lost..
Here is what I am hooking up to:
ESTP: Primary Limit Switch Input. This terminal should be grounded
for normal operation. When a Normally Closed Limit is opened, the
controller will send an Emergency Stop (E-Stop) signal to the
software. Interpretation of the g-code file will then stop.

Z Home: Z axis Home Switch input. When grounded during a Homing
operation, homing of the Z axis will stop.

Y Home: Y axis Home Switch input. When grounded during a Homing
operation, homing of the Y axis will stop.

X Home: X axis Home Switch input. When grounded during a Homing
operation, homing of the X axis will stop.

X Dir: X axis Direction signal. Connect to the X DIR line of the
stepper motor driver.

X Stp: X axis Step signal. Connect to the X STEP line of the stepper
motor driver.

Y Dir: Y axis Direction signal. Connect to the Y DIR line of the
stepper motor driver.

Y Stp: Y axis STEP signal. Connect to the Y STEP line of the stepper
motor driver.

Z Dir: Z axis Direction signal. Connect to the Z DIR line of the
stepper motor driver.

Z Stp: Z axis Step signal. Connect to the Z STEP line of the stepper
motor driver.

Fld: Flood coolant relay out. A logic high (+5v) is output when the M
code 'M08' (Flood On) is interpreted from the g-code file. A logic
low (0 v) is output when 'M09' (Coolant off) is interpreted.

CW: Clockwise Spindle relay out. A logic high (+5v) is output when
the M code 'M03' (CW On) is interpreted from the g-code file. A logic
low (0 v) is output when 'M05' (Spindle off) is interpreted.

CCW: Counter Clockwise Spindle relay out. A logic high (+5v) is
output when the M code 'M04' (CCW On) is interpreted from the g-code
file. A logic low (0 v) is output when 'M05' (Spindle off) is
interpreted.

Mst: Mist coolant relay out. A logic high (+5v) is output when the M
code 'M07' (Mist On) is interpreted from the g-code file. A logic low
(0 v) is output when 'M09' (Coolant off) is interpreted.

Lim: 2nd Limit Switch input. Use with Normally Open Switches. This
terminal should be left ungrounded for normal operation.

Gnd: Ground Terminals. All Gnd terminals are the same.

+5V: +5V terminals as needed. All +5V terminals are the same.

Any help would be appreciated!
