I found some free ones once upon a time for converting topo maps to
digital elevation maps, and for scanning topos into AutoCad for doing
survey work. They worked ok, but it wasn't ideal.

IM Service sells one called Rasterfratz. From the testing I did when it
was a beta product, it does an excellent job. It's not free, but it's
also not much.
