ready made ?
I hate to say it, but the instrumentation industry has these things
all over the place. the last time I looked one would go for about
$250.00 !

As Les said, an op-amp will work well.

Pretty simple to do if you want to do it yourself.

If you money is no object, but speed of delivery is, google signal
conditoners.

If you don't want to do it and can wait a week for delivery, contact
me off list. I have a friend who does this sort of thing all the
time.
