Addendum to my comments.
The instructions will only work with a WYE ("Y") connected motor with
a common brought out. It will not work with delta connected
motors. If you have a delta connected motor, you will have better
luck with a different motor unless you have the controller already.
If you have the controller and it takes 5 phases, a microcontroller
(even a Basic Stamp) is probably a cheaper choice than a new motor
and a new controller. Programming a Basic Stamp in their Basic is
pretty easy, you could do it in a logic array if you wanted, but that
might be no easier and the Basic Stamp might turn out to be cheaper
for no more than a three axis system.
