There are some excellent third party products that plug into Rhino and add full-featured CAM capability.
RhinoCAM from Mecsoft is one of them.

madCAM (as in "mould-and-die" CAM) is another.
http://madcamcnc.com
madCAM is especially well supported with a forum and direct access available to the developer. It's always advancing, currently moving fast in serious rotary axis support.
