Yes, this can happen from time to time. Usually caused by excessive load and vibration. You can make sure both the taper and
socket are clean and free of burs. Put the chuck back on the spindle, snug it up hand tite. Then whack it in place with a peice of
2x4 or other soft material. It should stay on, but it will pop off with excessive vibration or side loading.
