DC servo

HI all
i have a DC servo with gear box. it is a small motor. it is 1.5 inch in dia
length 3inch and gear box 2"X1.5". overall motor +gear box length is 4.5 inch.
it is from PITTMAN. USA
GM9412J818
12 V DC 218.4:1 RATIO
500 CPR (encoder plus/rev)
01-12-96

2 wires of motor and 5 pin plug for encoder. but 4 wires are in encoder
plug one is open.
encoder wires are these.
1 black
2 open(no wire)
3 yellow
4 red
5 blue


any one knows about encoder connections?
witch driver kit can be drive it?
