Very interesting... using a large diameter screw (say 12" dia) as a
base allows you to not only raise and lower an XY table, but also
act as a rotary table.

Incorporating yet another (opposite handed) large dia motor driven screw,
allows the table to rise without rotary motion of the upper XY table, or
all kinds of combinations if the two motors were independent.

This likely wouldn't be too fast in Z, but it would be very rigid.

Another option is a mini-hexapod bolted to the table. This of
course would give 5 axis CNC, but whether it could be made rigid
and small enough is an issue.
