I use Oldham couplings - see:

http://rswww.com

and search on "Oldham" to get an idea what they look like. The coupling
consists of 2 hubs coupled via a plastic torque disc. Should be able to
source similar items in the USA too.

Not too hard to make your own, but a little care needed machining the slots
in the disc if you are to avoid backlash.
