Hi Dave,
You are probably thinking of the charge-pump signal. It will output a steady
12.5kHz signal whenever Mach3 is running. If you're breakout board supports
it then use it, it's a good safety but is not mandatory for Mach3 to run
(although you're breakout board may need it) and it can be turned off to
free an extra output. (It's actually turned off by default, I beleive)
Now when you mentioned it I seem to remember something about a 'weak' output
or something but can't recall which pin it's supposed to be. Just try one,
I'm pretty sure you won't run in to any problems.

For reference, here's my complete pinout which works nicely:
Pin1 (Output) - ChargePump.
Pin2 (Output) - X-Dir
Pin3 (Output) - X-Step
Pin4 (Output) - Y-Dir
Pin5 (Output) - Y-Step
Pin6 (Output) - Z-Dir
Pin7 (Output) - Z-Step
Pin8 (Output) - Auxillary Output 1 (Flood)
Pin9 (Output) - Auxillary Output 2 (Mist)
Pin10 (Input) - X, Y, Z - Home (Prox switches via diodes, active high)
Pin11 (Input) - MPG Channel A
Pin12 (Input) - MPG Channel B
Pin13 (Input) - E-Stop (Ext E-stop pressed, active high)
Pin14 (Output) - Spindle Speed Step.
Pin15 (Input) - Available for future use. (Probe perhaps)
Pin16 (Output) - Spindle Dir.
Pin17 (Output) - Spindle On/Off. (Aux Output 3).
