mach3 or 4 is not realtime. The printerport module Art wrote is as close to realtime as you can get on windows.
what is the hardware on the machine? (steppers?)
my thought would be running Linuxcnc (it does run a realtime patched os)
inexpensive mini atx motherboard with printer port. (or a used decent computer <100)
some sort of stepper drive breakout board (I could not tell you what is good) Aprox $40
