Ken,
use the comment function to 'manually' add the required 'index' code
to the G code program, e.g. M81, or, you can have a 'post processor
function' which can be tailored to suit the output you require.
For example, some 4th axis, when used for indexing, also require
a 'clamp/unclamp' signal to be output.
