5-axis cnc

Does anyone know how to make the 4 & 5th axis for a cnc as found in:
http://www.rainnea.com/cnc.htm
Is there a kit for this? How about any www sites? Where did the
motors mount at for the 4&5th axis? Any suggestions or links
apprecitated.
