I would say, you would make the proper pattern in wax and investment
cast it. Either that or use two pieces and silver solder or bolt it
together. You aren't going to mill it with square corners inside unless
it is really big enough to get the mill head inside of the cavity.

Of course, there is a chance you could EDM it with the proper shaped set
of electrodes.
