Pin 2 is to ground, pin 1 is to about 470 ohms or so, which is then
connected to +5 volts, this puts 10 ma through the diode which should
be enough, a 1K resistor from pin 3 to +5 is fine, pin 3 becomes the
output (goes to logic 1 when at limit, beam interrupted), and pin 4 is
ground, or a separate ground if you want isolation. IF you want
isolation, then the +5 that the 1K resistor goes to is common to the
computer, and nothing else, the ground for pin 4 is the computer
ground.

Pins 1 and 2 can be fed from the 5 volts on the motor driver circuit.

This whole thing behaves like an opto isolator, so you get isolation.
