Silk-screenig is how the big boys do it. It starts the same way as making a paper copy - you just print it to a clear plastic, or have a print shop create the "film". Costs about $20.0 - $25.00 per film. It's a lot more to get a printing hosue to screen only one copy, but maybe the local art school is looking for a project?
If silk-screening isn't an option, you could print it to paper, have the local Kinko's laminate it, then punch the holes where needed.
