Bill is correct in what he says.
You have to remove the cover, remove the indexer board and fit
jumpers to the first five sets of pins from the top.
Like so

o--o
o--o
o--o
o--o
o o
o o
o o
o o
etc.
The jumpers are Samtec SNM-100-BK-T
I have a sheet here that shows all this that was supplied with a load
of 650 drivers I bought.
It's only a sketch but the description should suffice.
The rest of the wiring is covered in the manual Bill posted the link
to.
