I think the cutter must have an involute profile, which is easy enough
to approximate with a circular form tool on a lathe, maybe with the
cutter offset to provide some relief.
Also I don't think you need a CadCam application, it looks like you can
easily write it by hand. You will need to set the ratio of the x-axis
feed to the equivalent linear motion of the 4th axis to the same ratio
as the Tan of the helix angle, or maybe the ratio is the other way
round.
After the pass or passes index one tooth and repeat.
