It sounds like the 1PPR input isn't connected to the encoder counter.
Make sure that the 1PPR input is selected as "Spindle Index" in
stepconf. The other possibility is that if the spindle speed is too
high, then the index input could be missed. I don't think that's the
problem though, if it looks like the spindle-position signal tracks the
physical spindle rotation.
