A couple days ago I replied to your query but wanted to expand on a single axis motor with an integrated driver.
I suggested something like this (read below)but your torque requirements maybe higher than a motor like this can provide.

At the lin engineeeing website you can find all you may need under their "integrated motors" page.

I don't work for them, but I have been to their facility, seen programming of these stand alone motors drivers and have been very impressed. I am looking for a reason to buy one of these things to automate something at my work.
