You don't, you wind a clockspring with nonlinear torsion around it to make a linear/timepiece quality, actuator mechanism.

Or you make a screw for a plastic injection molding machine. Some times also used in Hydraulic pumps too.

That's the only applications that I have run into in the real world. Both usually have variable pitch AND variable pitch diameter.
