Thanks for the replies.
I asked Protel directly, and below is their response. Unfortunately,
the eval version of Protel does not include Camtastic so I may be out
of luck :(

Thank you for contacting Altium Total Support regarding milling. At
present, you can not mill in Protel, but you can in Camtastic.
Camtastic is
a gerber editor/viewer that is included for free with the Protel
Software
package.
The process is as follows:
First you will need to set up your drill tools. This is done by
zooming out
slightly from the layer you would like to mill/route.
Once you have zoomed out. Go to TOOLS> create drill(s), left mouse-
click,
and draw a box around your whole board so that it is highlighted and
then
right-click. This will set up the different tools that you will need.
Next you will want to select TOOLS>create route path(s). If you
would like
to select your whole board then highlight it, otherwise either
highlight the
desired section or select the path(s) individually.
This will create a new layer called (ie. Pcbcomp~rout)
You may need to clean up some of the mills. You can go about this by
using
the Editing features, under the main toolbar EDIT.
