You may want to check your SCSI buss termination. A lot of times SCSI
hardware errors look like software errors. Adaptec boards have a setup
screen that is usually available at boot time, or via the BIOS setup
screen. Assuming that your have one hard drive and one controller, make
sure the buss is terminated at the drive and the controller. Also, I've
had trouble with the Adaptec 'auto terminate' feature. I usually
disable this and manually terminate the buss.
