This is not that hard. You could cut the square depression a little
deeper than needed, and then lap the top face until the dimension
was exactly that desired. I did something like this in a part that
constrained 8 optical fibers in a groove .005" deep. I cut the
groove .006" deep, and then lapped the top until it was down to
.005".

Making punch dies this way is not so desirable beacuse the stresses
on the parts are huge, so a one-piece die is much better.
