Your last line re-posted below makes my point exactly! :-)

And the standard is NOT what has broken down... ADHERANCE TO and
updating OF the standard is what has broken down. While there ARE
places in the standard where misinterpretation COULD creep in (and
other PLANNED "holes", as mentioned before), they certainly do NOT
begin at G02 (the 3rd code)...
