Well, that certainly applies to machines where internal service is not
frequent. But, it doesn't
make much sense to lock out and tag out a milling machine every time you
need to wipe some
oil on a workpiece, and then endure a one hour procedure to restart the
machine. When doing
one-off type work on a milling machine or lathe, the operator or
machinist needs to be
in the work area constantly to brush chips, change tools, inspect the
progress, etc.

And, there's no doubt that putting your hands, or even your head(!),
inside the work area
of such a machine is a hazard.
