When in a industrial/shop/commercial environment, they make multiple
hole pad-lock plates.

When I did some work a duPont, to get a piece of equipment locked
out, we had to get the supervisor to place the plate and put his lock
on it. then the electrical forman put his lock on it. then my
supervisor put his lock on it, THEN I was allowed to put MY lock on
it !

Needless to say, machines do not self start.
