The cheapest ball screws I've found are at http://www.mcmaster.com
Look at the 5/8" diameter, it is the least expensive. You can also
consider acme screws, for which http://www.mscdirect.com is less
expensive. Ball screws are more efficient at transferring power than
are acme screws.

As for slides, eBay is probably the best bet, unless you have the money
to buy new. The company I work for uses 1/2" Thomson linear bearings
and (supported) round ways. They work a long time with no trouble. We
also use THK for some applications, again, they work a long time with
no trouble.

As for what exact size to use, can't help you there. Search the web for
other people's machines, and also plans, kits or complete machines, and
check what they're using for slides, motors, etc.
