Could be your work surface is not flat ?
are the problem holes always in the same area ?
Or your gantry / pantograph is not at absolute right angles to the
worksurface.
Easy test for all this - just clip a dial gauge to your Z axis
and move it around the table - keeping a good eye on the readings.
