I can answer part of this. A good photo editor like paint.net (free) can
crop to a mask or a shape. Once you export this, you can import to
Autocad, scale and move it to 0,0. I'm betting this can be done in free
CAD like Draftsight, as well.

What ever you are using for CAM should be able to select a point as
origin when making tool paths. If not, you can always offset the part on
the machine that will produce it. In G-code, you could use a G52.

Free advice is worth every penny.
