The first solution requires a custom lead sxrew...the screw must have a
right hand thread on one end and a left hand screw on the other end.

The second solution uses a hollow ball screw (these are available...i didn'r
say they were cheap) with and adapter mounted on one end. the adapter is
driven by a motor mounted 90 degress to the axis of the screw. this adapter
provides liner motion of the table thru the ball screw mounted on the table.
Extra linear travel is obtained as the adapter rotates on a secondary
threaded rod. A commercial part for making this work is available from
Dynamic Automation, 307 North Michingan Avenue, Suite 915, Chicago Ill.
