The other possibility (other than missing step) to consider is that a
sprocket or coupling might be slipping on a shaft. Set screws are simply
not reliable... although I've got two machines full of them at the moment.
Locktite is my friend, but I'm sure it will fail someday when I least
expect it - need to redo everything right.
