Looks like the belt/pulley match is wrong. The belt and pulley V should
match and give even wear on the side of the belt and except for "vari speed"
type pulleys, the belt should not be deep in the pully. It should be near
flush.
Check the angle of your pulleys, they should be the same on both and the
belt should have the same angle.
