to really help you out one would need more information about your
setup. The drive train ratio, the CPR of the encoders, friction
factors, etc. Given a full list of your system may enable someone
to locate where or what is causing your drives to fault.
Since you said "fault almost immediately", sometimes a simple
reversal of motor connections will work, but that is just conjecture
at this point.
