If the only objection is the requirement to have the spindle running when a
speed change takes place, the speed control stepper/servo could be
controlled by an auxiliary contact on the spindle motor starter. You would
have to have the setup "enabled" when speed change was taking place. The
back-gear speeds would still be manually accessed, but I would put a
microswitch behind the selector handle to reverse the motor direction back
to original CW/CCW by having a relay switch two of the spindle phase legs.

I had thought about this as well. There are issues with the controller
though - you have to be sure that the crank is only turned when the
spindle is running, for instance.

Another option is to use a DC motor and drive, like this guy did:

You get full torque at all speeds, so that's not a problem. You don;t
get "extra" torque on the low end though, so it may still me a good idea
to have changeable gearing.

I'm definitely curious about answers to this question as well.
