There are several sources. I know of Rockford, Thompson, & Hiwin. You
can also puschase through McMaster Carr or similar industrial supply.
What type of milling machine? I this for manual operation or are you
aiming at doing a CNC retrofit? For our class of machines, hobby, you
probably want "precision rolled" screws with anti-backlash nuts. These
aren't much more expensive than plain rolled but are somewhat more
accurate. I think they are just selecting the better pieces from stock
for this. Ground screws are conisderably more accurate but way more
expensive. The ground screws, at least on my old worn mill, would be
overkill and a waste of money.
I looked around for an X/Y ball screw retrofit kit and finally purchased
from Rockford. They supplied a kit for my Supermax mill rather than
having to slightly modify a Bridgeport kit. Also, after looking around,
I just ordered a screw and nut from them for my Z axis retrofit from
them.

It'll give you a good idea of what is avialable but no prices. They've
been good to deal with and will give quotes over the phone.
