HI Bill,
It looks like a single channel of opto, so it's probably a single index
hole.
There is a PDF manual on the board.

Probably about a #40 drill bit.
Some have put a pulley on the tail shaft of the Sherline, and ran a short
belt to an encoder device. For just a disk, I'd make up a hub with a
setscrew, and solder a .063" thick disk (3" diameter?) to it.
Look at the output of the board with a 'scope. You can always make the hole
a little larger. If you had the data sheet for the opto-interrupter, you
could look up its detector size for a suggestion of the hole size.
Make your own!
