Is the propensity for passing spikes due to the direct connection of the
"primary" and "secondary" commons? Or some other factor?

You're right, the variable transformers are (usually) made with just one
size of wire, but the principle of 'being careful' still holds for variable
x-formers. My preference for using variable transformer would be to use it
to 'feed' a properly sized isolation transformer. Also, an autoformer of any
kind is more susceptible to passing powerline spikes into the (so-called)
secondary. This can really screw up sensitive electronics.
