Mine also and thousands out there. The erickson QC 30 needs to be clean
and checked for worn parts. The nut needs to be adjusted and once worn past a
certain point, replaced. Check that the spindle taper is not damaged or worn.
Even a new nut won't keep the holder in under those conditions.
