I have put a picture at
http://www.spiyda.com/Images/leadscrewextension.JPG
with some blocks of color to show the arrangement of the bearings.

I used Skate bearings because they are so cheap..
so far, they have performed flawlessly, as well as proper thrust
bearings would have done.

BTW .. one advantage of extending the shaft is that the leadscrew nut
never gets close to the bearing, making alignment less critical.
