The files that I generate open in ViscamRP, and look correct so I'm
fairly sure I know what I am talking about.

The word contour means the same as the word boundary in this case, a
line that encloses something. So you might say there is a layer
header followed by a series of boundary lists. The first boundary
(what you insist on calling a countour) DOES NOT have to be the
outside of the object. Besides there may be multiple bodies on a
given layer.

If the direction is CCW it is outside.

The first boundary for a given layer can be internal or external it is
abitrary. If CCW it is an external one if CW it is internal. I know
this because the files I produce can produce the boundary lists in any
order.
