Hi Paolo,
a varistor in the mains would allow you to test, but like you said,
not every test finds every bug.
at lower voltage/current, you might not have passed the jiggle test.
Jiggling every part, wire, connector belt and pulley might have
exposed that poor connector.
and regarding design, if everything is working perfectly, you haven't
added enough features yet.
bty, do you put in led's across the fuses ? makes it easy to spot a
bad fuse and offers you that warning that the circuit is hot.
